An integrated assessment that goes beyond depression checklists, measuring functioning, quality of life, relationships, agency, and wellbeing: identity after football, grief about leaving the game, family strain, readiness for work, sleep and pain management, and sense of future direction.

Refer

A personalized Game Plan connecting the player's aspirations to vetted providers, trauma-informed therapy, financial counseling, vocational direction, substance-use support, leadership development, and family reconnection. We act as the navigator most players never receive.

We follow up, adjust referrals, document impact, then step out and let the provider relationship continue. The structure stays lean and scalable.
